The request goes into your own Microsoft 365. It is held in the front desk's Outlook calendar and added to a list the team works from, and the person who asked gets an automatic reply. Your team confirms the time the way it does today.

3DH Consulting is my company. I'm Portuguese and have lived in Warsaw since 2009. I ran businesses first, then spent three years at Accenture on the Microsoft account.
I build websites and the Microsoft 365 setups behind them for small companies. Recent website work includes the site of a renewable energy developer, in two languages, and the site and shop of a skincare brand.

Questions
It is $2,500, excluding VAT: one fixed price, agreed in writing before any work starts. No hourly billing, and no surprises at the end.
It helps, and you may already pay for it with your email. I'm a Microsoft licensing partner. If you don't have it yet, setting it up for up to five people is included; the licenses themselves are a separate monthly cost.
The form asks only what the front desk needs to call back: name, contact details, the reason and preferred times. It tells people not to include medical details. Requests are stored in your own Microsoft 365, not in a separate booking service.
Yes. The new site goes live on the address you have now. Your email is not touched, and your Google listing keeps pointing to the same address.
Yes. You review the whole site on a private link during the week and ask for changes. Nothing switches over until you say so.
Everything happens by email and video call. Your morning is my afternoon, so calls fit in before your first patient or at lunch.
